Phyllis Anderson was rappelling from the saddle of Twin Tumbs and Peak 11 when a piton came out. She fell 30 feet and landed on her pack (that was on her back), then started to slide down a 30 degree snow slope above cliff faces. Her partner, Barry Bishop was able to grab the rope she was trailing and arrest the slide. She was later carried out to the train.Source: Accidents in North American Mountaineering, 1954

Mark Chwirka was ascending "Slingshot Pinnacle" on a CMC trip in No-Name basin when a handhold came loose about 400' up the rute and he impacted the rock face .
